Brokers Report:

73 y/o female non-smoker. Medical history includes hyperlipidemia, reflux, and osteoporosis Borderline hypertension was also noted in '01-'05, but readings were better in '06. No meds noted for this so far. There were no current BP readings. She apparently had cataract surguries in 12/02 and 10/03, but not clear. Records from Dr Pacropis were illegible, and mainly consisted of various test copies and labwork. The only narrative notes included were from '02 and '03 (visits for surgery clearance), so no good details on overall medical history, although she appears to be doing well. Records from Dr. Smith end 6/07, where it was noted that her last colonoscopy was in '02 (no details on the results), and she was to check with GI doctor on when a repeat is due.

Need current blood pressure readings, names of all meds currently taking, and would obtain GI records. I would also confirm that the surgeries in 12/02 and 10/03 were for cataracts, and obtain copies of narrative notes from office visits over the past 5 yrs from Pacropis (if possible). Case may qualify for preferred rates so far, subject to missing info.
